• You have been prescribed Doxofylline for the prevention and treatment of asthma and chronic obstructive pulmonary disease (COPD).
• It should be taken at the same time each day, preferably in the evening after food.
• It does not work right away and should not be used to relieve sudden breathing problems. Always keep a fast-acting (rescue) inhaler with you.
• Your doctor may take regular blood test to monitor potassium level and the level of this medicine in your body.

✔️ Pharmacology

Doxophylline is a new type of bronchodilator distinguished by its structural variance from Theophylline, featuring a dioxolane group at position 7. This compound selectively hinders phosphodiesterase 4, resulting in the relaxation of bronchial smooth muscle. Notably, unlike Theophylline, Doxophylline displays reduced affinities for adenosine A1 and A2 receptors, contributing to its enhanced safety profile. Additionally, Doxophylline has been observed to inhibit platelet-activating factor (PAF) and the generation of leukotriene production.

• Adult Dose: Varies based on formulation, typically 1-3 times daily. Dosage adjustments may be needed for elderly patients or those with specific medical conditions.

Fixolin should not be combined with other xanthine preparations. It is advised to moderate the intake of beverages and foods containing caffeine while using Fixolin. Caution should be exercised when administering Fixolin concurrently with ephedrine or other sympathomimetic drugs. The simultaneous use of certain medications such as erythromycin, TAO, lincomycin, clindamycin, allopurinol, cimetidine, influenza vaccine, and propranolol may decrease the hepatic clearance of xanthine derivatives, leading to elevated plasma levels of the drug. Phenytoin, along with other anticonvulsants and cigarette smoking, may increase the clearance of xanthine derivatives, resulting in a reduction of their plasma half-life. Adjustments to the dosage of the drug may be necessary in such cases.

Animal studies have indicated that the active component of Doxophylline does not disrupt pre and postnatal development. However, due to insufficient clinical data on the effects of the medication during pregnancy, its use should be assessed on a case-by-case basis, considering the risk-benefit ratio. Doxophylline is contraindicated during lactation.

Several factors can decrease the hepatic clearance of xanthine derivatives, leading to elevated plasma levels of the drug. These factors include age, congestive heart failure, chronic obstructive pulmonary disease, severe liver disease, concurrent infections, and the coadministration of certain medications such as erythromycin, lincomycin, clindamycin, allopurinol, cimetidine, influenza vaccine, and propranolol. In such cases, dose reduction may be necessary. Monitoring of blood drug levels is advised when factors affecting xanthine derivative clearance are present to ensure therapeutic levels are maintained.

Exercise caution when administering the product to patients with cardiac disease, hypertension, the elderly, severe hypoxemia, hyperthyroidism, chronic corpulmonale, congestive heart failure, liver disease, peptic ulcer, and renal impairment. Particularly, use caution in patients with congestive heart failure, as drug clearance is significantly slower, leading to prolonged high blood levels even after treatment cessation. There is no risk of addiction or dependence with this medication.
